    • Purpose of collection: to provide products or services to you to fulfill our contract, to process your payment information, arrange for shipping, and provide you with invoices and/or order confirmations, communicate with you, screen our orders for potential risk or fraud, and when in line with the preferences you have shared with us, provide you with information or advertising relating to our products or services.

    • Disclosure for a business purpose: shared with our processor Shopify, our shipping app PirateShip, and our fulfillment company Sigma Services.
    • Personal Information collected: name, billing address, shipping address, payment information (including credit card numbers), email address, and phone number.
  • Veterinary client information (if applicable)
    • Purpose of collection: to affirm a veterinary client-patient relationship has been established and provide the veterinarian associated with the order an opportunity to review the product(s) being shipped to their client. 
    • Source of collection: collected from you.
    • Disclosure for a business purpose: shared with the veterinary office noted in your order
    • Personal Information collected: client name, name of dog(s), email address, phone number, and veterinarian contact information

Please keep in mind that if you directly disclose personally identifiable information or personally sensitive data through Puppylation Health's public message boards, this information may be collected and used by others. 

Use of your Personal Information 
Puppylation Health collects and uses your personal information to operate its website(s) and deliver the services you have requested. 

Puppylation Health may also use your personally identifiable information to inform you of other products or services available from Puppylation Health and its affiliates. 

Minors
Puppylation Health does not knowingly collect personally identifiable information from children under the age of thirteen. If you are under the age of eighteen, you must ask your parent or guardian for permission to use this website. Behavioral Advertising
Puppylation Health may keep track of the websites and pages our users visit within Puppylation Health, in order to determine what Puppylation Health services are the most popular. This data is used to deliver customized content and advertising within Puppylation Health to customers whose behavior indicates that they are interested in a particular subject area.

  • Temporary blacklist of IP addresses associated with repeated failed transactions. This blacklist persists for a small number of hours.
  • Temporary blacklist of credit cards associated with blacklisted IP addresses. This blacklist persists for a small number of days.

From time to time, Puppylation Health may contact you via email for the purpose of providing announcements, promotional offers, alerts, confirmations, surveys, and/or other general communication. If you would like to stop receiving marketing or promotional communications via email from Puppylation Health, which you can unsubscribe from by clicking the link at the bottom of the email. We share your contact information with Shopify, our email marketing provider, so they can send these emails on our behalf.

We may email you with messages about your order or account activity. For example, we may email you to tell you that:

  • You’ve created a Customer Account
  • Your Customer Account password has been reset or updated
  • You’ve made a purchase
  • Your veterinarian has been contacted or has reviewed your order 
  • Your order has shipped

It’s not possible to unsubscribe from these messages.

We share your contact information with Shopify, our website hosting provider, so they can send these emails to you on our behalf.

We may store your data on servers provided by third party hosting vendors with whom we have contracted.

Changes
Puppylation Health reserves the right to change this Privacy Policy from time to time.
