Our products are designed to be used with the guidance of you dog's veterinarian. Once your order is placed, we will contact your veterinarian for approval.

If you have more than one dog, separate each name with a comma.

If the clinic does not have your name on file, please enter the name of the dog owner the clinic has on file.

We'll let the vet's office know it's a gift from you to their customer so they'll know to keep it a surprise.
